How are these attached to the conduit? I'd assume they are threaded inside?

Yes they have female threads.

In the picture they are upside down. Although they can be installed either way even sideways. But If you're going in the can from underneath, you ussually would not use one.

I agree for a meter enclosure or a panel. If they're going into a junction box then one (or a sealing locknut) would be required on the bottom as well.
