It is a knockout seal that was held in place by a screw run through a breaker blank up.
I kinda liked it!

1 1/2" Push-in KO Seal with crushed tabs and a 1/4x20 hole drilled in the center and a CH Breaker filler plate tapped by a 1/4x20x1/2" screw to cover a 1 1/4" KO
