installing an isolated ground

This is a question relating to a flex whip at a data processing center. This is the way I see this installation. The green wire with yellow stripe goes to the recepticle and the solid green wire goes to the box and flex. The termination point is at a power distribution unit. There is a separate ground bar at the bottom of the panel, for the green with yellow stripe, that is insulated from the pdu and then is electrically connected to the equipment ground buss at which point you would terminate the solid green wire. The problem is I have run across an installation at this facility that had the green with yellow stripe connected to the box and flex. This seems backwards to me but being Primarily an industrial electrician I am not very familiar with this type installation. Is there an industry norm to this color code or does it a vary? I would appreciate any information with this type of installation. Thanks In advance.
