How would one prevent the sections from unscrewing when you have to reverse the drill for one reason or another?

In residential, self-feed bits work with minimal hangups. The bits are usually used for plumbing vents but work well for in-wall riser masts through plate lines.
I have never tried using threaded pipe because most distances can be drilled with (3) 16" extensions. I used a stick of emt and expansion adapters for 1" bit with a 1/2" drive Hawg through balloon framing fireblocking once and it worked OK. Got lucky....

In reference to the AEH, I agree that it may be okay for technical references, but for code exam there are better books.
