Small issue in the scheme of things but it irritates me no end. Junk never ends. Fortunately it didn’t short out & didn’t have a live red buried in the armor. Seen that a few times too.

It is an MC connector but not a very good one. I replaced it with another one.
 
I am having a hard time seeing what's wrong with this particular connector.
It appears to me that the problem is with the installation. If the connector will grip the sheath firmly, it could have been tightened with the cable at the proper depth.

Yes, installer was part of the issue.
