HVAC maintenance attached fixture with self tapping screws and my concern is if the ballast shorts, wouldn't the duct work be the conductor and injure someone down the line?

The fixture and the duct should be grounded. So if the ballast shorts to either one, the current flow should trip the circuit breaker. Fixtures are attached to metal items often, so I don't think you need to worry about the duct shocking someone.

However, I think Pierre has a valid point.

Lots of metal duct work remains without intentional grounding, mostly of the fan coil type but not limited to it. The reason is the unit and it's duct system are seperated by an insulated material to reduce noise transmission.
