Is there a code for lockout tag key.Who should have the keys.Should the supervisor have one or should only the person working on the issues.

NFPA 70E PartI, Chapter 5 in General. Section 5-3.3.7 specifically covers your question.

A lock out lock should only have a single key and that key should remain in the possession of the worker at all times. It is his protection.
