Wow. 411.4 changes quite a bit in the 08'. We're still in the 05'. Edit* Whoops. scratch that.

Because 411.4 tells us to use a chapter 3 wiring method if concealing the wires in a wall. To me, that means all of Chapter 3 applies. Yes/No?

I vote no.
 
So- lets say yes you need to wire according to chapter 3- which ends up meaning you need access to all splices.
Then comes the definition of the accessibility again.
Say your doing recessed pucks-
Pull the fixture down- bam you have accessibility.
Right?

Wrong. The connection is concealed and is not a chapter 3 method.
 
There was a good article posted on here a while back regarding LV splices not in a box and how they can be a fire hazard. Unfortunately I cannot find the article or the link, however I do have the text of the article, which I cut and pasted to show to an architect. It's too long to post here, but if anyone is interested give me a shout and I will em it to you.

Interestingly enough if the wiring method is exposed then the splice does not need to be in a box. The infamous puck lights are perfectly legal used with the system provided as long as the wires are not concealed.
