Please be as specific as you can. Your questions appear to have a common thread, but I am unable to get enough from them to be of any assistance. Thanks. :-? Also, it has been stated here on this forum that the NEC is not intended for untrained persons as per 90.1(C) and your questions point toward unfamiliarity with 3 phase motor wiring methods. Maybe someone will help you find the proper electrical training books such as "Stallcup's Generator, Transformer Motor and Compressor" book, or any number of other helpful manuals such as those available on Mike Holt's site, right here.

Is this motor a part of a machine, for example, a dial index machine?

If so, the wiring within the machine itself is generally not subject to the NEC.

Most machinery manufacturers these days use some kind of cord for wiring of smaller motors, sometimes even putting a connector at the motor so the motor can be removed without having to unwire anything.

Cords tend to get less practical as the size of the motor gets larger.

I suggest you get a copy of NFPA79 - 2007 Electrical Standard for Industrial Machinery. NFPA also offers training courses for this Standard.

This code often overrides or supercedes certain NEC requirements.
