Hey Guys, I'm installing a sub panel for a customer so I can also install a generac transfer switch. The customer has a milbank 200 amp combo meter panel. I going to relocate the 12 circuits to the new sub panel and install a 200 amp breaker in the existing milbank combo panel. The existing combo panel has siemens type qp breakers in it. What model 200 amp breaker will fit in this combo panel?

No, I'm relocating the 12 circuits in it and installing a 200 amp breaker something like the picture I've attached. I'm trying to find the exact model that will fit.
That one will not do well, in a older Speedfax it was stated they were for for multi metering equipment & 300-400A loadcenters, wiring bending space will be a big issue with them.

It's either a milbank or midwest meter/panel combo. There are 6 (2 )pole breakers in it now, no 200 amp main breaker.I'm adding a 200 amp breaker to the combo panel and using it to feed a new generac transfer switch which will the feed the new sub panel where I will relocate the 6 (2) pole breakers from the old combo meter/panel.

I don't recall ever seeing a meter combo without a MB. If this does really exist I doubt a 200 amp branch breaker would be a option.

Six service disconnects were permitted at one time. I would doubt that a panel made for six disconnects would have a provision that would permit the installation of a 200 amp main.

My point exactly.
Why not? Sounds like a MLO panel and he wants to install a 200A branch. As long as there is no specific restriction listed on the thing whats the problem? Its probably a Siemens Q2200B

Just curious, if this is a MLO, typically most of those double poles are things like range, dryer, water heater.....don't you NOT want those on the generator?
The customer purchased a 22 kw generator, it'll run everything she has provided she doesn't run everything at once, which is what I preach to all me generator customers.
