Look at 230.71. The Main Breaker is usually present to serve as a disconnect for the service. If there is a disconnect ahead of the panel (it is installed as a subpanel) and the disconnect provides overcurrent protection for the feeder, then you may use a Main Lug panel. Usually (and this is an oversimplification) Main Lug panels may only be used as sub-panels.

So say you have a shop that you are going to feed with a 100 amp breaker from the house, the sub panel at the shop can be a 100amp main lug panel.

A sub-panel in a separate structure would require a disconnecting means. Typically a main CB is the simplest way to accomplish this.
 
I am not sure of prices in your area but here in ohio 100 amp 20 space main lug sub panel is around $45.00, the 100 amp 20 space main breaker value pack with 5 20 amp breakers is $59.00 I just buy the value pack you will need breakers anyway.
