Mandarory Grounding Bushings reference

IEEE STD 142-1991, Grounding of Industrial and Commercial Power Systems, paragraph 2.1.3. notes that grounding bushings are mandatory for services and systems over 250 volts to ground, per the 1990 NEC.

I have searched the 2005 NEC Handbook and can't find the requirement.

Can anyone help? Thanks.

Look in bonding around 250.94 and above.
