Hello fellas:

I have an existing installation, and I want to adapt it for a medical office. The NEC in section 517.13 requires me to install an additional conductor for the isolated ground. Can this conductor be added in the same wiring route, or do I have to change all the wiring that goes to each receptacle?

Note that actual "isolated ground" receptacles are only allowed in limited areas of a medical facility (seen 517.16). They would be allowed in the office although that requirement is now rare,
As far as the "redundant" grounding for receptacles in patient care spaces (a wire type equipment ground conductor in a metallic raceway), you can add the an equipment ground to an existing raceway as long as you don't exceed the fill requirements.
Redundant grounding is not required for office areas.

You're misinterpreting what 517.13 says. 517.13(A) requires the raceway to be a recognized EGC per 250.118, 517.13(B) requires an ADDITIONAL wire type EGC (think redundant) as a back up in parallel with the raceway. As Augie said, isolated grounds are pretty much not allowed in healthcare.
