What should the height of the meter box be from the ground surface? Where is this referenced in the NEC? Where should the measurements be taken?

The NEC does not reference the meter can. The meter can belongs to the Utility company. In my area its 4' to 6' to center of can , from grade level

Oops,my bad Atricle 312 does reference meter sockets. Need to check with your utility company for heights

Every electric utility has their own set of rules. What do you do for a utility that you would not know the answer to your own question? :D
