Meter Disconnect required.

Goodmorning All, In Mass. When we are required to upgrade a customer's service entrance riser on residential we have to change to a meter/main Disconnect, to replace old meter enclosure, ? Is if there is no need to change the existing panel in basement which is in good shape and has a main Breaker I do need to replace the existing 3 wire SE is deteriorated, can I just replace that with 3 wire SE if I am keeping the existing panel in tact. Or must i change out the cable to a 4 wire now that the 1st disconnect is with the Meter. And if I must change to a 4 wire now the existing panel becomes a sub panel I must seperate grounds and neutrals, but now my 3 wire existing electric range and dryer's do not comply possibly. Can I get some info on the code actual requirements with article references if anybody can help. Thanks

Someone posted the other day that in MA the emergency disconnect is only required when replacing the entire service.

However, when the emergency disconnect is required, it does'nt automatically require changing to 4 wire.
