Well I just starting watching this DVD last night and it was very enlightining. Interesting how lightnining produces ans expanding/collapsing magnetic field and induces voltages into the electrical wiring thus the reason for grounding.

So a ground fault is when a hot/neutral touches a box, raceways etc.?

No, not exactly. The inductive effects of lightning are mitigated through bonding practices, not grounding. While grounding is an essential component to a lightning protection system and premises wiring systems, the connection to earth is not necessary to mitgate the inductive effects of a lightning surge event.

Per the 2011 NEC:

Ground Fault. An unintentional, electrically conducting
connection between an ungrounded conductor of an electrical
circuit and the normally non?current-carrying conductors,
metallic enclosures, metallic raceways, metallic equipment,
or earth.
