Pix #3, I'm going to say the bolts are not threaded all the way up, so they installed washers so they could tighten the nut down.

Washer between the lug and bus, causing the lug to not make proper connection to the surface of the bus.

The lugs are not making proper surface connection to the bus, which can under load create excessive heat.
I have not seen an installation like this before.
