Yes. I don't see an IR image to go with each pix. :grin:

Also, the equpment seems to have a lot of loose red arrows floating about. You might want to vacuum those out.;)
 
Well, let's see.

Pix #1 is obvious. Someone forgot their crimp tool that day.

Pix #2, hard to tell if it's not a Bellevue washer.

Pix #3, I'm going to say the bolts are not threaded all the way up, so they installed washers so they could tighten the nut down.

That is what it looks like. They could have installed the washers on the bolt head side between the head of the bolt and fitting and installed the the compression fitting tight to the surface or installed proper sized bolts.

The lugs are not making proper surface connection to the bus, which can under load create excessive heat.
I have not seen an installation like this before.

Number one you need a code reference, for this. Please do not apply for a jonb here.

2 is a listing issue bolt size and hole size, plus the washer.

3 is just commmon sense and may be a listing issue as well.

Belleville is the type of washer. Bellevue is a mental hospital in NY. Please elaborate.

110.3(B) is the first section I would refer to. The manufacturer's product instructions would be next.

As I have been taught, the surface of the lug is to make contact with the bus, not a washer. The amount of surface area has been computed and designed by the manufacturer, UL and others. That info is found in the product standards and instructions.
