When do you need to install a motor starter and heaters on a motor and when can you just install it on a breaker ? What determines this ?

You never need a starter, but the motor always requires overload protection. In general if the motor does have built in overload protection, you need some type of external protection.
 
What about a 1.5 HP pool filter pump motor 12 amp with internal thermal protection?

No starter needed if it has internal thermal protection.

It does not matter whether it has internal thermal protection or not. A starter is never required. However some means of overload protection of the motor is required and often this is supplied as part of a starter.
