Multi-family load calculation non-continuous?

Is a residential load calculated using demand factors continuous or non-continuous? For example would a calculated load of 390 amps required a 100% rated 400A main circuit breaker?
If the answer is yes I would probably just go to a 600 amp service.
 
The process of calculating the load will take into account the possibility that some of the load may be continuous. You don't take the final calculated value and add another 25% to that number.
