I was asked to do a review of a Medical Building which has multiple clients. The service has a group of six meters and disconnects outside and MLO panelboards inside. Should those panelboards have main breakers? What code do I refer to?
 
Each panel has to be protected from overcurrent by a device within the panel or a device upstream. Reference NEC 408.36. You described the upstream stuff as "meters and disconnects." Is there also a fuse associated with each meter, or each disconnect? If I correctly understand what Bob said, the answer is generally that they do. But if not, then you will need breakers within each panel.
