Multiple wires on each lug of a panelboard main breaker?

I saw an interesting two conductor, one hole lug a few years ago that I have never seen since. It had a setscrew that clamped the first conductor, than the second conductor went over that and was clamped between that and an outer set screw. IIRC it was an 80's 400 amp square D switch.

I've seen several of them, think they maybe even were older then 1980's but maybe were still in production into the 80's. Surprisingly never seen one that became loose and overheated, and most I have seen have probably been improperly used with more conductors then originally designed for.
