Need Help on Combo Meter / Main / Breaker Panel

After all of 5 months on the job I have one I just can't figure out. The 2008 book gives me clues and a search on this site gives me clues...but I still don't think I have the answer.

The dilemma is a new lake home wiring job. There is a pole mounted combination meter socket / main breaker (200 amp) and breaker panel currently mounted. (All In One Unit) This has been in place for a approx. 4 years.

In the existing breaker panel on the pole is a 2 pole breaker feeding a water well and a 50 amp 2 pole breaker feeding an existing garage sub-panel appox 10 feet from the pole, plus a utility outlet fed by a 20 amp GFI breaker mounted right below the breaker panel.

I'm installing a 150 amp panel with main braeker in the new building. The question is ?? How to feed it ??

I'm leaning towards a 4 wire feeder, but given that this combo panel already exists, do I need to put a 150 amp 2 pole breaker in the existing panel and feed the new building 150 panel from that breaker?

I know this is another 3 vs. 4 wire questions with the added bonus of figuring out if I bond on don't bond in the new panel. Some previous posts come close to answering this, but I have this added caveat of the existing breaker panel on the pole.

What is a fella to do???
