N2deepsky, don't know if will help but here goes. On a 240/120V 3ph4w Delta Service one way of avoiding having to deal with the 208V High-Leg we install two panels for branch circuits, one is 240V 3ph3w (NO NEUTRAL) and the other is 120/240V 1ph3w (WITH NEUTRAL). If you look inside the 3phw4 panel the manufactures listing will show both 3ph4w and 3ph3w applications.

On a grounded system a grounded conductor must be brought to the service equipment as has already been stated, this is purely for clearing faults if the case is that all circuits are phase to phase or line to line.

After the service equipment an EGC will be doing the job of fault clearing so the grounded conductor is not required if it is not needed for carrying current as part of a circuit.

A sub-panel supplying phase to phase or line to line loads is no different than a typical residential water heater, no neutral needed.

Design iincluding a grounded conductor is a different issue but it is not a code issue.
