New outdoor fused service disconnect and service/feeder sizing

Dealing with somewhat incomplete plans and unresponsive engineer, need to confirm a few things here quick- appreciate all help in advance:

Plans call for installing new service conductors to a distribution panelboard in an existing wellhouse -120/240V 3 phase.

1. New fused service disconnect at the pole will be 3-pole, 3-fuse with solid neutral & suitable for use as service equipment & install my MBJ here (Per 250.24 C)?

2. Cable schedule calls for "4 - 500KCMIL" from the pole disconnect to an existing panelboard in wellhouse. However with no other info provided, I'll just assume size the new fused main disconnect 400A rated & protect the ungrounded feeder (no longer a service) conductors with 400A fuses?

3. Underground run between service disconnect at pole and existing panelboard is now a feeder (as the fused disconnect at the pole is my service disconnect now) , so I need to add a #3 ground per table 250.122?

#1) Sounds good
#2) 500 kcmil=380 amps, good with a 400 amp OCPD if the calculated load is 380 amps or less.
#3) Yes the feeder requires an EGC. Could be metal raceway or according to T250.122 a #3 Cu conductor as you've stated.
Thank you for the sanity check. Boiler plate specs specify conduit shall not be used as an EGC, so I'll pull a #3Cu.

it's 200' from the pole to the existing panelboard, not sure why they'd want the service disconnect at the pole, but that's what they're indicating..
