New Service from Meter at Power Pole

I am tasked with the design of a 300-400 amp service for a 6000 ft new build. The power pole with the transformer and the meter is 150 feet away. I don't know a plausible installation with a disconnect at the outside of the house or at the power pole. Two 200-amp feeds keep entering my mind, but I think it has to be a 300-400 disconnect.

You could install 2 sets of service conductors off the load side of the meter to two grouped 200 A disconnects on the outside of the house (assuming you need an exterior disconnect, if not you could put them inside nearest the point of entry), see 230.40 exception #2

I think I can come out of a 320 meter w/parallel conduits to the house w/200-amp disconnects.
 
You don't mention which edition of the NEC you need to meet. 225.30 in the 2017 would limit you to a single feeder. That changed in the 2020 to allow multiple feeders.
