new service grounding and bonding

new 200a underground service with a main breaker in the meter and a 200amp main lug panel in the house. i connected ground rods to grounding lug in meter than ran a 4/0 ser service cable to my main lug panel. in my main lug panel i seperated ground and neutrals. i ran #4 copper from lug on ground bar of main lug panel to water meter. in one town i passed and one i failed the one i failed said water ground had to go back to main disconnect. i am a little lost cause we have a ground in the service cable and it connects from meter to panel so why cant i tie my water ground to panel.

250.24 Grounding Service-Supplied Alternating-Current Systems.
(A) System Grounding Connections. A premises wiring system supplied by a grounded ac service shall have a grounding electrode conductor connected to the grounded service conductor , at each service, in accordance with 250.24(A)(1) through (A)(5).
(1) General. The connection shall be made at any accessible point from the load end of the service drop or service lateral to and including the terminal or bus to which the grounded service conductor is connected at the service disconnecting means.
Yep that's the way I see it.

250.52 Grounding Electrodes.
(A) Electrodes Permitted for Grounding.
(1) Metal Underground Water Pipe. A metal underground water pipe in direct contact with the earth for 3.0 m (10 ft) or more (including any metal well casing effectively bonded to the pipe) and electrically continuous (or made electrically continuous by bonding around insulating joints or insulating pipe) to the points of connection of the grounding electrode conductor and the bonding conductors. Interior metal water piping located more than 1.52 m (5 ft) from the point of entrance to the building shall not be used as a part of the grounding electrode system or as a conductor to interconnect electrodes that are part of the grounding electrode system.
I think this implys that both GEC and the MBJ has to be at the same point

I know that whenever you have the panel down stream from the disconnect, it must be fed with a cable or raceway containing a seperate ground and neutral conductor.
Where, in the code, does it tell us to do this? My best guess?? is 250-142 (B). Is it stated any more clear elsewhere?

I don't mean to be a nit picker but there is no standard definition of the term sub panel. The only place that a connection is supposed to exist between the neutral and the Equipment Grounding Conductors is via the main bonding jumper at the service equipment.
