no equipment grounds

I have 2 meters and 2 disconnects someone took the feeders from panel B and hooked em to panel A load side of the disconnect so they had one bill for the 1 store. Now its back to being 2 stores so All I have to do is put the wires back done. but.... This is a really old service 30 years or more and there are conduits going down under building and straight up into panels. This is 3 phase 208 w/neutral. The conduit is metal and I noticed there is no separate equipment ground conductor or a bonding lock nuts. The panel is hooked up with neutral and grounds on the same bar. For all I know its direct burial wire and no pipe under the building. I am going to have to get this inspected. Im in Jersey. (Rehab Subcode.) And was wondering if this is just simply moving the wires and landing them where they have been inspection back in the day or should I be prepared to add bonding locknuts and seperate grounds in the panel. Someone took a screw out of the 3 phase 100 amp fused disconnect so instead of finding any old steel screw that fits I was going to but a 3 phase main breaker service disconnect in place of this. Because the fused ones cost more otherwise I would put a fused one in. Maybe the Disconnect should not be there because the wires enter immediately from the ground below I could just put a main in the panel then. but I want to just swap disconnect and be OK. Is this a legal install from way back? if seen it in older strip malls. and old equipment.
