odd installation, Code violation??

I saw this install where someone previously had run a multiwire branch circuit, ( hots and 1 shared neutral together), the on one of the breakers they installed antoher wire (basically 2 wires on 1 breaker) and ran another neutral for this for this 1 extra hot wire on this breaker .I coudnt think of a code violation right off had (other than maybe the breaker wasnt listed for 2 wires) but it seemed very odd.

Anything wrong with this set up??

Yes they are protected by a 20 amp breaker, its just that they have the 2 wires under 1 breaker, but one of the wires are part of a multiwire branch circuit, and the other is not, so the need for the extra neutral.

Just seems odd they did it this way, but i can seem to think of a code violation of this set up.
 
circuit problem not an issue.
Is the breaker rated for two wires? I think square d QO is the only one rated for this. I could be wrong.
