I have Three 40 ft, 3 class poles in aligment, the middle pole is going to have a 10kva transformer install on it, does the pole need a guy wire installed as well.

Ask your local utility! Unless subject to a high wind gusts, guying seems unnecessary.

It depends on several factors. Wire span lengths, Wire size, wind loading and ice loading(depends on where you are). You probably will not need a guy.
