Can someone name this panel and how it works ? Ive heard the terms feed through lugs or something like that
The supply side or line side has lugs with space for 2 wires how does this work or what is this used for ?

It's a feed-through panel, probably between floors. The incoming and outgoing feeders land on the double lugs that were installed to accommodate the paralleled feeders.

The 3-pole breaker is merely another breaker, possibly supplying another panel or a transformer.

Why would someone need 2 feeders in one panel ? I understand the load side supply power downstream but why the double lugs on the line side
 
Which are you calling line and load? Unless you know, you can't tell here which way the power is going.

Granted, it's likely going up, but the incoming and outgoing are the same size and ampacities.

The same wires are joined in the next panel(s) up and down, so consider them as single larger wires.
 
To add, those pairs of conduits do not branch out in different directions.

Now I'm questioning the diagram from what I see a 3 pole breaker is feeding the second the panel but now why does the second panel have 2 black 2 red 2 blue going into the main breaker
Because they're effectively the same thing as double lugs, whether on a breaker or panel terminals.

Two wires make up each phase instead of larger single wires of the same total equivalent ampacity.

The second picture is just to explain how feed-through panels are just a continuation of the feeder.
