Plastic bushing requirements by voltage

Our plastic bushings required by voltage used. I was under the understanding it was required after 250 volts to ground or higher. Is that correct or where do I find that? Thank you

Bushings of some sort are required on raceways with a #4 or larger conductor unless the fitting has a smoothly rounded surface and also required on rigid and IMC of any size. See 300.4,342.46 & 344.46 for details.

It must be, but I thought there was also a requirement for plastic bushings at voltage as well. Thank you

Either an "identified" smoothly rounded insulating surface, or a "listed" smoothly rounded metal surface. No voltage restrictions in 300.4(G).
300.4(G)...
(1) An identified fitting providing a smoothly rounded insulating surface
(2) A listed metal fitting that has smoothly rounded edges ....
