Portable Generator Connection

We have a portable 45 KW generator, being used to feed a 208/120 VAC 3 ph-4 wire 100A MCB panel. Via a manual transfer switch. Circuit breaker on the generator is 150A, 3-pole. Customer wants a pin-sleeve connection - I assume I need a 150A because of the generator breaker? Is there a way I can credit the MCB in my load panel and use a 100A plug/receptacle?
 
thanks, that was my read as well, but was checking to see if there a code allowance I was not seeing.
 
Leaving the portable generator out of the equation for a minute, installation of a 100amp inlet to a 100 amp mts to a 100amp mcb panel is all ok right?

Now is the generator actually 45kw or is it 45 kva?
