I saw a reference on a sheet to a 12 cal/cm PPE. The 4 categories are 4,8, 25, and 40. Where is 12 coming from?

The categories you referenced are for when PPE is selected using the appendix tables.
When calculations have been performed you are allowed to select PPE based on the actual incident energy. For several NFPA 70E cycles there has been a appendix showing a simplified PPE selection based on 12cal/cm2.

I know my old company switched to 12 cal/cm2 as our minimum wear almost 10 years ago. This level probably covered 80% of the locations I visited.
