There is an outfit in Pa Who makes concrete pole bases to your specifications. ,If you need more info let me know and i will get you a contact number of their pricing and availability. We have used them before and their product is affordable and suited our needs.

Most concrete companies have a 2 yard minimum. However, there are companies that charge you by the yard and they mix it right there on the job. I call those guys the "Willy Wonka Concrete Factory" because of the way the machine sounds when it mixes. " EEP OOP AANT!" They pull right up to the base and let her rip. Rebar, wood for bracing the base, soni-tube should all be considered as well as an auger truck to dig the hole. Get quotes for your materials first and then you can base your labor on your experience with this type of work. Driving a ground rod to ground the pole does not provide a low impedance path back to the source to open the OCPD in case of a fault. With a properly sized equipment grounding (bonding) conductor, a ground rod at each pole can be used, but I see no advantage to driving one at the pole except for lightning protection.

Prefabricated Bases

If you have more than one or two bases, an auger truck and a prefab base beat pouring them any day.
Check your Bluebook for a local Brooks concrete products branch and see what they have to offer.
I have been in Florida for seven years now and have not seen a single base poured on site yet.
Before that, I had never seen a prefab base.
The fixture supplier usually supplies engineered drawings for permitting.

We always let the concrete company do their work.We install the anchor bolts and conduit and on site does the concrete.Next day we install the poles.
Labor per pole and 4 heads $850.00 not including the conduit and wire.

shouldn't concrete work be done by licensed concreters?

Yeah, and the digging should be done by licensed diggers, too.

Looks like The State is ignoring the needs of those workers by failing to protect their industry. When any ol' electrician can do what they do and get away with it, it can't be good for society. :)
