Rockwell Automation has announced a significant PSA on select 140M-C*, 140M-D*, 140MRC*, and 140U-D* that were manufactured in August 2014 and September 2014 (date codes IB (August 2014) or JB (September 2014)). In a small number of instances, when the breaker is in the OFF or TRIPPED state, the identified breakers may unexpectedly or intermittently conduct electricity thru a single phase. This can result in voltage being present on the load side of the breaker with the breaker in the OFF or TRIPPED state.
Shock, vibration, or other environmental conditions may cause an affected breaker to unexpectedly conduct electricity after the breaker has been placed in the OFF or TRIPPED state, even after voltage has been verified to be absent on the load side.
