Raceway grounding conductor

Obviously plastic conduits such as pvc would need a grounding conductor and some flexible metal conduits may need one depending on the length and ampacity of the conductors.

If you are asking about emt or rigid (rmc) then no it is not required but often spec'ed by engineers
 
Has it ever been code that you need to run a ground wire in all conduits

Only by local amendment. I used to live in Oregon, and the area I worked in mandated a wire EGC in all conduits. Their reasoning was that it is an earthquake zone, and they didn't want a pulled-apart conduit coupling breaking ground continuity in an earthquake; they wanted the OCPD to operate to shut off power quickly. I really couldn't argue with that logic. I'll still pull the green wire on a case by case basis, but not all the time.

Glad I don't live out there any more. The scenery was nice, but working with ignorant lunatics got old pretty quick.

One of the three counties I work in requires a ground wire in metal pipe by local amendment. I have to remember to put it in when I'm working there.

Around here (MA) all new jobs have a EG wire. been that way for probably 30 years. Not code required.

I think it's a good idea, especially in a ceiling area quite common to find EMT pulled apart from being stressed. If I am rewiring and older job and the pipe is strapped and supported well I will sometimes skip the ground wire
 
If you submitted a code proposal to require an EGC in all EMT runs, the CMP answer would be "the product meets standards if installed correctly".
And testing shows that EMT is a much better EGC than a wire type
