That looks like a depiction of a recessed fixture with integral mounting clips not a box.

My first thought too, fixture.
He could gang two together, didn't see 4sqr. just 4".

Raco and others make the box below and then there are madison clips and other items that you can use to keep it in the ceiling.

I will tell you that it is not compliant to hang fixtures from this type of box in our area. Not sure how your authority having jurisdiction rules on it.
