Not to familiar with residential 3 HP fire pump systems.
1. Do rules from NEC 695 apply?
2. Should the OCPD be ahead of the main as per 695 or is it allowed in the a panel?
3. I didn't find 13D helpful... any other codes which may help me out?

13D is the document you need. The pump does not need to be wired ahead of the main disconnecting means. If you look at the 2013 edition, you'll see that the only requirement (electrically) is that the pump be connected to the 240 supply (6.2.1(2)). If this were a 13R system, then the requirements of NFPA 20 would apply.
