Service entrance conductor meter pedestal disconnect requirements

Can anyone find the code section (2011 would be fine) that states the maximum distance a service entrance raceway may be installed before a meter disconnect ( fused on non) is required. Here's the catch. The raceway is 36' and installed under a concrete slab?

I do not think that you will find it in the NEC. There are limits on how far service conductors (i.e. those with no overcurrent protection) can be run inside a building, which vary locally based on the AHJs interpretation of "nearest the point of entrance".

If it is under enough concrete, it is officially considered not to be inside the building.
