When is is required to install a duct bank for conduits under the building slab? Does it have anything to do with the spacing of the conduit? Is it o.k. to install conduits on top of each other without any spacing?

I think we should clear up what you veiw as a 'duct bank'

In my experaince that genrally means a group of conduits installed on racks with enginered spacing requirments encased in concrete

You already know the answer to that, you must maintain spacing.

How much space is up to debate but IMO it is always more than none.
