I just put appliance cords on the DW and the disposal. Both of these machines will need replacing some day and this makes the next guys job easier. It also is a cleaner looking install. I'm not going to worry if the cord is approved for that appliance or whatever. This is one area of the code I just ignore.

You can ignore any area of the code if you wish - but what does your inspector ignore, or at least what is he/she ignorant about? If there is no inspector then you are taking any liability on yourself for doing it by your own code. I do this myself sometimes, but I find myself more often than not deciding to do things according to recognized standards (like NEC) so that if something should go wrong I am not to blame for doing it 'my way'.

We probably take on liability even on jobs that are inspected. I don't think the fact that a job passed inspection is going to protect me much in court if a code violation that the inspector missed causes harm.

I agree it won't and it doesn't.

If we install a code violation today it is ours until it is corrected or we die.

At best an inspection shows that you try to follow the rules, if that actually changes the result of a legal action I have no idea.

What do you think will happen to your liability insurance premiums if the insurance company found out that you do not follow recognized codes and standards, but instead had your own codes and standards?

One thing about the NEC is it is made by contributions from anyone. Getting your contribution put in there is not easy, but should not necessarily be easy. There have been in the past sections that have everyone wondering what the CMP was thinking, often those sections are changed the next time around.

What gets me is you can go to Menard's or Loews and buy a UL listed for Dishwasher/Disposal 14/awg pigtail cord with a molded plug for about $3.00

So why does UL on one hand want "the right cord" for a dishwasher and on the other hand list a universal cord for the same purpose, If this ever came up in court I would bet I would have an easy case with UL playing both sides of the street like this.
