In the NEC 250.122, Table 250.122, how is the size of the grounding conductor determined/calculated based on the protection rating?

It comes right out of the table.
A 15A OCPD requires a #14 copper, etc.
There are some cases where it has to be made larger, but this is the smallest it can ever be for a branch circuit with that specific size of OCPD.
