The biggest smoke escape that I ever saw was when some company electricians were wiring up a water treatment pilot plant in a 45 ft trailer. They had installed a 100 kVA 3-phase transformer in the MCC to run a dozen motors totalling about 70 HP. The wiring was mostly in trays that had conduit off to each motor.

When they flipped on the main disconnect, smoke started rising from the trays for the whole length of the trailer. The insulation was melted on some wires in all of the trays and conduits, and the whole thing had to be rewired. They never told us what they did wrong.
