Hood exhaust fans in restaurants, bathroom exhaust fans in office buildings, and roof top heating/AC units with VAV boxes.

I would not consider any of these continuous duty.

An exhaust fan system in a traffic tunnel, might be an example. Indefinitly long time !!

Is it not the case in Art 430, the term "continuous duty" has to do with the motor marking per 430.8 and not the 110 definition of "continuous".
Unless the motor is marked for less than continuous duty as identified in 430.22(E), is it not treated as "continuous" in Art 430 (regardless of it running or not running 3 hrs) ?
 
Aside from the motor load, you have the heater load, plus the spa-pack load which typically includes a blower motor, ozonator, and lighting. All of that is going at one time or another.
