Sub-contractor safety pre-qual.

Does anyone have a good safety pre-qualification form that is specific to trenching/excavation sub-contractors. Most of these guys are small contractors and don't think OSHA applies to them. I want to increase their awareness.

Just do a online search and you will get more then you need. Many insurnce compnaies will have generic forms that you can customize.
