You can pretty much put it where you want but the closer the better. You would need some kind of conduit unless you mount it right on the panel.

Why don't you just use the breaker style unit that just installs in the panel?
I have seen the Square D ones, but do the other manufacturers make them too?

Absolutely, yes, individual wires must be encased.

The ones that mount to the panel typically have a 1/2" nipple attached to them that goes in a 1/2" KO and is secured by a locknut.

Wire length and 90° bends can negatively impact the performance of surge protectors. Follow the manufacturer's recommendations.
