I see three code violations in this panel.

1. Bottom right 2 pole breaker feeds a white and black wire. The white wire should be marked with some tape to show as a current carrying conductor.
2. Looks like a ground was too short to reach the ground bar, so a splice using a wirenut was used to extent the wire to reach the ground bar. No splices permited inside a panel.
3. Conductor fill on the branch circuits?

Electrify, splices are allowed in panels. 312.8

I would say

1)Using the bare for a neutral

2)Running a separate EGC from the cable assembly

3)The unmarked white on the 2 pole breaker.

...and the sheathing for the 10/3 doesn't enter the panel by 1/4".

I knew something was up with that SER, couldn't quite see it clearly. Good onion, Bob. :D

*Bonks head with palm of hand* Wow, I just noticed that the service wire needs another conductor for 3 phase. Man what a hack..I hope thats temperary lol.

Isn`t that a double lugged neutral 4 th from botom,The 2 pole is not remarked as a current caring condductor,splices are permitted in a panel as long as thier is room.
On a scale of 1>10 for neatness I `d say a 3

MA for romex is not approved for this use.Also site that taped bare for neutral.And the ungrounded wire on breaker need tape.As for neatness i give it a 2 and thats pushing it.

Since this is a 3 phase panel and it has NM in it, does this suggest the type of structure and another possible code violation.

Hint: This job was never inspected.

I see 2 black wires under breaker #4. Are these breakers capable of landing 2 conductors?

How long are those PVC sleeves? Is this installation (running NM through PVC) even accepted anywhere? Shouldn't these NM cables be clamped?

Well not to me as we often install NM into three phase panels and switchgear. :D

Whom ever wired this Panel I would consider "Dangerous". To take SE wire and use it in a manner it is not made for shows improper training. Nor is there any consideration for his fellow electricians. It's sad that their are people out there doing this kind of work.

The pvc is 74" and extends above the drywall ceiling that is in the equipment room only. The entire building is wired in NM including the fluorescent lay-in fixtures in the typical suspended panel ceiling.

Maybe the guy was from MA, we can still use NM for this. :p

So Dave what are the three violations in the panel.

Jim, this looks like a pretty neat installation to me. Let's see some pics of your work. Wouldn't it be a waste of time and therfore money to do anything neater than this? That's what I have heard.

Has the lug on the right been tightened yet?
