I have never seen one and I am certain one does not exist.
You probably have pointed out a mistake in the code book? One that most people would overlook?

I would be willing to bet there was at some time.

Larry what you betting on ??

You are on let the bet start

Yeah many " bleu moons " ago there were few 115 v 3 phase motors that was used in early to mid 20 th centery as soon the voltage of the power system rise and the 115 v 3 ph motor just fell out of the flavour just like 115 -230 volt single phase motor still around but the winding connection was done the same way in triphase way back that time but i dont rember how popuar it was that era majorty of them useally wired 230 or higher volts .

But i am not sure if i am mistken but i know some industrail users did use the 115 v motor that ran on 25 HZ but i might be wrong on history wise but it was listed in the nec code book for quite a while [ i think they did use simuair design in european as well for short while too ]

Merci , Marc

They should address this code book issue before the new book comes out. In my opinion that first column in this table should be removed.
