Wonder if all three transformers were wired wrong, and in the same way.

The OP stated (5) xformers not (3). Highly unlikely that wrong wiring is the cause. I suspect it to be some kind of device such as a TVSS (SPD) mounted inside of each Xformer and may not be easy to find especially if they are oil filled
